Sciatica vs. Herniated Disc: What’s the Difference—And Why It Matters

Understanding Your Back and Leg Pain in Dayton, Ohio

Spoiler alert: One causes the other. But they’re not the same.

If you’ve been searching for answers to sharp lower back pain, burning down your leg, or nagging hip discomfort, you’ve probably come across the terms sciatica and herniated disc.

You might be wondering:

❓ Do I have one? Both? Neither?

❓ What’s really causing my pain?

❓ And what should I do about it?

Let’s break it down—clearly, simply, and from a spine expert’s point of view.

Sciatica Is a Symptom. A Herniated Disc Is a Cause.

Think of it like this:

🟧 Sciatica = What you feel (nerve pain symptoms)

🟩 Herniated disc = What’s happening in your spine (physical structure)

You can have sciatica without a herniated disc.

You can have a herniated disc without sciatica.

But often, they’re connected, and understanding how is the key to real relief.

What Is Sciatica?

Sciatica refers to a set of symptoms that occur when your sciatic nerve, the largest nerve in your body—is irritated or compressed.

Common signs of sciatica:

✅ Sharp or burning pain in your lower back, buttock, or down the leg

✅ Numbness or tingling that travels from your spine into your foot

✅ Leg or foot weakness

✅ Pain that worsens when sitting, coughing, sneezing, or standing up

💡 Important: Sciatica isn’t a diagnosis—it’s a nerve pain pattern.

It’s your body’s way of saying: “Something’s pressing on a nerve.”

What Is a Herniated Disc?

Your spine has soft, cushion-like discs between each bone. When the inner gel-like material pushes out through a crack in the outer shell, it’s called a herniated disc, also known as a slipped or ruptured disc.

If this bulge presses against a nearby nerve (like the sciatic nerve), it can cause sciatica.

But not all herniated discs cause symptoms. Some are found on MRI by accident, with no pain at all.

Herniated Disc ≠ Always Sciatica (and Vice Versa)

Here’s where it gets confusing:

A herniated disc can cause sciatica.

But sciatica can also be caused by other spine conditions, such as:

  • 🌀 Spinal stenosis (narrowing of the spinal canal)
  • 🔩 Spondylolisthesis (slipped vertebra)
  • 🦴 Bone spurs
  • 🔍 Rarely, tumors or infections

That’s why personalized diagnosis matters.

How Do We Figure Out What’s Really Going On?

At Vertrae® in Miamisburg, we listen before we treat.

We take time to understand:

  • When your symptoms began
  • How the pain travels or changes
  • What movements make it better or worse
  • How it’s affecting your job, sleep, or daily life

Then we use imaging (like MRI or CT scans) to match your symptoms to the structure of your spine. Not the other way around.

🧠 It’s not about treating a scan. It’s about treating you.

Treatment Options for Sciatica or Herniated Disc

We believe in a conservative-first approach. Most patients get better without surgery.

Non-Surgical Treatments:

✅ Targeted physical therapy

✅ Anti-inflammatory medications

✅ Activity modification

✅ Epidural steroid injections for pain relief and clarity

What If Conservative Care Doesn’t Help?

If your symptoms persist or worsen, minimally invasive spine surgery may be the next step.

Surgical Options Include:

🔹 Discectomy – Remove the part of the disc pressing on the nerve

🔹 Laminectomy – Create more space by removing small bits of bone

🔹 Barricaid® Implant – Seal the disc after discectomy to prevent future herniation

🔹 Spinal fusion – Stabilize the spine if it’s unstable

🔹 Disc replacement – Preserve natural movement by replacing the disc

Your spine solution will depend on your anatomy, your activity level, and your goals.

At Vertrae®, we walk you through every option clearly, without pressure.
