Real Relief, Not Just a Patch: How Spinal Cord Stimulation Offers Hope for Chronic Back Pain

A Natural Next Step for Patients Still in Pain After Surgery

In our recent blogs, we explored why some patients continue to experience pain after spine surgery: what’s commonly called Failed Back Surgery Syndrome (FBSS) or, more accurately, Persistent Spine Pain Syndrome. We then outlined a full spectrum of diagnostic tools and treatment options designed to uncover the true source of pain and treat it at the root.

Now, we turn to one of the most effective and evidence-backed treatments for patients with nerve-related pain that persists despite prior interventions: Spinal Cord Stimulation (SCS).

If you’ve exhausted other options and still haven’t found lasting relief, this could be your turning point.

What Is Spinal Cord Stimulation?

Spinal Cord Stimulation is an FDA-approved, reversible therapy that uses a small implantable device to send gentle electrical impulses to the spinal cord. These impulses interrupt pain signals before they reach the brain, reducing or even eliminating the sensation of pain.

Key Facts:

  • Involves thin wires (leads) placed near the spine and a pulse generator (like a pacemaker).
  • Targets nerve-based pain often resistant to traditional treatments.
  • Helps many patients reduce or eliminate the need for opioid medications.
  • Proven effective for:
    • Failed Back Surgery Syndrome (Persistent Spine Pain Syndrome)
    • Complex Regional Pain Syndrome (CRPS)
    • Chronic sciatica or nerve pain
    • Diabetic peripheral neuropathy (see our companion blog for more)

How Does It Work?

The concept is based on the gate control theory of pain: by sending safe electrical pulses, the device “closes the gate” to incoming pain signals. Some patients feel a mild tingling (paresthesia), while others use modern paresthesia-free systems that are entirely sensation-free.

You feel less pain. You move more. You get your life back on track.

Two Steps to Relief: The Trial and the Implant

Spinal cord stimulation is not an all-or-nothing decision. It begins with a temporary trial.

Step 1: Trial Phase (1–2 weeks)
  • We place temporary leads using a needle and connect them to an external device.
  • You go home and “test drive” the stimulator.
  • If you experience 50% or more relief, we move to the next phase.
Step 2: Permanent Implant
  • The leads are secured under the skin.
  • A small battery is placed in a pocket under the skin (often near the hip or lower back).
  • You can adjust your settings at home using a remote or smartphone app.

If the trial doesn’t help, we remove the leads. No harm, no commitment.

What to Expect After the Permanent Implant

  • Most patients go home the same day or after one overnight stay.
  • Recovery takes about 4–6 weeks with some activity restrictions.
  • You’ll have regular follow-ups to adjust the device and monitor your success.
  • Most patients return to their usual activities—with fewer pain flares, better sleep, and lower medication needs.

Are There Risks?

Yes, as with any procedure, but most are manageable:

  • Infection at the incision site
  • Lead movement requiring adjustment
  • Minor discomfort or tingling sensations

At Vertrae®, we minimize risks with meticulous technique, pre-op preparation, and postoperative monitoring. And since the device is removable, you’re always in control.

The Results Speak for Themselves

✔️ Up to 70% pain reduction in clinical studies

✔️ Dramatic improvements in quality of life, sleep, and daily activity

✔️ Better long-term outcomes than repeat surgery for many FBSS patients

SCS doesn’t just mask the pain. It helps your nervous system regulate itself.

Not Everyone Is a Candidate, and That’s a Good Thing

One of the reasons Vertrae® patients have such strong outcomes with SCS is our careful screening and patient selection.

Ideal candidates often have:

  • Nerve-related leg or back pain that’s lasted 6+ months
  • Pain that hasn’t improved with medications, PT, or injections
  • Prior spine surgery with persistent symptoms (FBSS)
  • Clear imaging and clinical correlation

We rule out SCS when:

  • There’s an active infection or bleeding disorder
  • Mental health conditions may impair success
  • Structural issues need to be addressed first

Why Vertrae® for SCS?

Vertrae® isn’t a general pain clinic. We’re a spine-only center of excellence with a multidisciplinary team focused on lasting outcomes.

  • Here’s what makes our approach different:
  • Expert evaluation by board-certified neurosurgeon Dr. Kamal Woods
  • In-depth diagnostic testing to confirm the source of pain
  • Coordinated care between your surgeon, PT, and pain specialist
  • In-house physical therapy and advanced neuromodulation support
  • Access to the latest SCS technologies, including burst stimulation and closed-loop systems

Most importantly, we support you through every step—from pre-trial counseling to long-term device management.
