ReActiv8 Explained: Restoring Stability for Chronic Mechanical Low Back Pain

When Your Back Pain Just Won’t Break the Cycle

Some people have back injuries, undergo physical therapy, and gradually move on.

Others don’t.

At Vertrae®, we meet patients every week who say things like:

  • “My back goes out with the smallest movement.”
  • “PT helps… until the next flare.”
  • “I feel off. I feel unstable.”
  • “Something deep just isn’t working.”

If that sounds familiar, the problem may not be your disc, your nerves, or your joints alone.

It may be the multifidus, the deep stabilizer muscle that quietly protects your spine every time you bend, lift, twist, or transition from sitting to standing.

And when the multifidus shuts down, the entire system becomes vulnerable.

ReActiv8® is an FDA-approved therapy designed specifically for this problem. It doesn’t mask pain. It doesn’t burn nerves. It doesn’t fuse the spine.

Instead, it restores control to the stabilizing system that never fully recovered after injury.

For the right patient, it can be a turning point, especially when back pain keeps returning despite doing “all the right things.”

Quick Snapshot: Is ReActiv8® Right for You?

  • Treats chronic mechanical low back pain from multifidus dysfunction
  • Restores muscle control, doesn’t block pain signals
  • Best for patients with:
    • 6+ months of deep, midline, or near-midline pain
    • Minimal or no leg symptoms
    • Failed PT, injections, or medications
    • MRI evidence of multifidus atrophy or dysfunction
  • Preserves motion and improves control; not a quick fix, but a long-term solution

“ReActiv8 restores spinal stability by retraining the multifidus muscle, treating the root cause of chronic mechanical low back pain rather than masking symptoms.”

What ReActiv8® Actually Does

ReActiv8® is a restorative neurostimulation system. It doesn’t just block pain. It rebuilds stability.

That distinction matters.

Rather than masking pain signals, ReActiv8® delivers targeted restorative neurostimulation to the lumbar medial branch nerve, producing repeated, controlled contractions of the multifidus muscle. Over time, this retrains motor control and coordination, helping the stabilizing system relearn how to function automatically again (Deckers et al., 2018).

Unlike spinal cord stimulation, which alters how the brain perceives pain signals, ReActiv8® works at the source of mechanical instability. It gently and repeatedly activates the nerve that controls the multifidus muscle, allowing the muscle to re-engage and do the job it was designed to do.

Think of it less like a pain treatment and more like physical therapy for a muscle that can’t be reached voluntarily.

Patients use the device at home, twice a day for about 30 minutes. Each session triggers a controlled, non-painful muscle contraction, just enough to retrain timing, coordination, and control.

Over time, this can lead to:

  • Better segmental stability
  • Fewer sudden flare-ups
  • More confidence in movement
  • Less guarding and fear
  • Reduced medication use

That’s why ReActiv8® is motion-preserving. It doesn’t shut down the system. Quite the opposite- it brings it back online.

Why Multifidus Matters More Than Most People Realize

One of the most important things Dr. Woods shares with patients is this:
“Your back didn’t fail. The system that stabilizes it never fully came back online.”

After an episode of disc pain, facet irritation, vertebrogenic inflammation, or even spine surgery, the body often responds by instinctively shutting down key stabilizer muscles—especially the multifidus. This is the nervous system’s way of protecting an injured area.

Research has shown that this protective response can lead to persistent inhibition and atrophy of the multifidus muscle after low back pain, contributing to impaired spinal stability and a higher risk of recurrent symptoms (Hides et al., 2001; MacDonald et al., 2009).

The challenge is that this shutdown can happen quickly… and it doesn’t always reverse itself once the pain fades. Even when the original pain generator improves, the multifidus may remain offline.

Physical therapy helps many people restore this control. But in long-standing cases, especially when MRI shows multifidus atrophy or fatty infiltration, the muscle doesn’t always restart on its own.

That’s where ReActiv8® comes in.
It doesn’t just suppress pain. It helps restart the system.
Restore activation → regain control → rebuild stability → reduce flare-ups.

This isn’t about numbing symptoms. It’s about bringing the spine’s natural support system back online, so your body can do what it was built to do.

Who ReActiv8® Is (and Isn’t) Designed For

ReActiv8® May Be Right for You If You:

Best Candidates

Not Ideal If…

Have 6+ months of low back pain

Pain is dominated by leg symptoms

Pain worsens with bending, sitting, or movement

You have severe spinal stenosis or a large disc herniation

Pain feels midline or near-midline

You need surgical fusion for structural instability

Have failed conservative care (PT, meds, injections)

You have an active spine infection or severe bone disease

Show multifidus atrophy on MRI

You require urgent decompression

Patients often tell us:

“I don’t need something to numb it. I need my back to feel stable again.”

That’s exactly what this therapy is designed to support.

What the Research Shows and Why It’s Different

ReActiv8® isn’t new or experimental. It’s been backed by years of solid research showing that patients experience real, lasting changes.

Across well-designed randomized controlled trials, patients treated with ReActiv8® demonstrated significant and sustained improvements in pain, physical function, and quality of life, with benefits maintained at both two- and five-year follow-up when used in appropriately selected individuals with chronic mechanical low back pain (Gilligan et al., 2021; Deer et al., 2021).

What stands out most is how those improvements behave over time. Instead of fading, as so many temporary treatments do, outcomes with ReActiv8® tend to build gradually.

That’s because this isn’t an instant fix. ReActiv8® works by helping the spine’s stabilizing system turn back on and stay on. Just like any true rehabilitation process, retraining coordination and control takes time.

The upside? By addressing the root problem rather than masking symptoms, the results are designed to last.

What Recovery Actually Feels Like

Most patients describe recovery in phases:

Early (first few weeks): Learning the stimulation pattern, less guarding, and subtle improvement.

Middle (2–6 months): Easier transitions, fewer “oh no” moments, improved tolerance for daily activity.

Later (6–12 months): A sense that the back feels trustworthy again, something many patients haven’t felt in years.

ReActiv8® isn’t about instant relief. It’s about restoring confidence in movement.

Why ReActiv8® Fits the Vertrae® Philosophy

At Vertrae®, everything starts with one principle:

Preserve motion. Restore stability. Treat the root cause.

ReActiv8® aligns perfectly with that approach because it:

  • Doesn’t remove anatomy
  • Doesn’t burn nerves
  • Doesn’t limit motion
  • Doesn’t commit patients to fusion

Instead, it strengthens the system that protects motion.

That’s exactly the kind of solution we look for when patients want durability, not just relief.

How We Know If ReActiv8® Is a Good Fit for You

At our Vertrae® spine center in Dayton, we thoroughly evaluate every candidate.

When we’re considering ReActiv8®, we take a close look at the full picture:

  • What does your MRI show about muscle quality? (Are there signs of multifidus atrophy or fatty change?)
  • Are there signs of disc or endplate irritation like Modic changes or degeneration?
  • Does your pain behave more like mechanical instability or inflammation?
  • How does your body move during the exam — do your stabilizers activate when they should?
  • What have you already tried, and why didn’t it work long term?

We only recommend ReActiv8® when the pattern is clear:

  • The multifidus isn’t doing its job
  • Physical therapy was done properly, but didn’t fully solve the issue
  • And there aren’t other pain generators (like a herniated disc or SI joint dysfunction) overshadowing everything else

This kind of careful matching is how we protect your outcomes and your trust in the process.

Final Thoughts: What If the Real Fix Is Stability?

ReActiv8® isn’t for everyone, but for patients whose back pain keeps coming back despite doing everything right, it can be a turning point.

It’s not just about strength. It’s about restoring the coordination and control your spine needs to stay stable.

If flare-ups keep happening, it may be time to look deeper.

Let’s find the right next step.
